OZONE AS A MODULATOR OF THE IMMUNE SYSTEM

Abstract

In  order  to  clarify  the  immunomodulating  properties  of  ozone,  we  have investigated: a) the effects of stimulation on isolated peripheral human blood mononuclear cells (PBMC) from normal donors with either ozone or ozonated serum; b) the range (in terms of O3 concentrations) of the therapeutic window; c) the stimulatory and toxic effects and d) the pattern, of both proinflammatory and immunosuppressive cytokine production up to  86 hours after exposure to O3. Results show that ozone can act as a weak inducer of cytokines producing IL-6, IL-4, TNF-α, IFN-γ, IL-2 and IL-10 and, most importantly, there is a significant relationship between cytokine production and ozone concentration. Analysis of the proliferation index  shows that progressively increasing O3  concentrations inhibit IP and therefore appear cytotoxic.

Introduction

Leukocytes comprise a heterogeneous cell population composed of lymphocytes (20-25%), monocytes (about 5%) and three type of granulocytes of which the neutrophils are about 70%. We have been the first to show that an appropriate ozone dose can induce a small release of  interferon γ (IFN- γ) from human blood (Bocci and Paulesu, 1990). Later on the number of cytokines has expanded to IFN-β, interleukin 2 (IL-2), IL-6, IL-8, tumor necrosis factor α (TNF-α),  transforming  grow  factor  β1 (TGF-β1)  and  granulocyte-monocyte  colony stimulating factor (GM-CSF) (Paulesu et al., 1991; Bocci et al., 1993a, b; 1994; 1998a, b). Later on several authors (Beck et al., 1994; Arsalane et al., 1995; Jaspers et al., 1997) have confirmed that ozone can induce the production of cytokines after that epithelial cells of the respiratory mucosa have been in contact with ozone.

Our results were obtained by ozoning blood directly and cytokines were detected in the plasma during the following 4-8 hours of incubations. These initial studies shed light on several aspects such as the protective effect of blood antioxidants, the dissimilar production of different cytokines and the progressive inhibitory activity of increasing ozone concentrations, particularly above 80 µg/ml per ml of blood. However they had limitations because firstly, whole blood can be incubated only for a limited time and, most importantly, we could not decide which cell type produced the cytokines.

During the last year we decided to isolate from normal blood donors either peripheral blood mononuclear cells (lymphocytes and monocytes, PBMC) in order to investigate their viability and the production and type of cytokines released after two different ozonation modalities. The first is a direct ozonation of PBMC suspended in human serum, so that cells undergo the total action of ozone due to immediate effects by hydrogen peroxide  (H2O2) and other unidentified reactive oxygen species (early ROS), with very short half-life, and late effects, provided by lipid oxidative products (LOPs), with fairly long half-life. The second approach has examined the effect of ozonated serum 20 min before addition to PBMC and therefore ozone activity is expressed only by “late LOPs”.

 Abstract

Blood samples of 15 patients with threatened abortion, 15 healthy non-pregnant women  and  15  pregnant  women  without  pathology  were  investigated.  The following indices were determined: the antioxidative plasma activity (by method of  bio chemiluminescence),  the  level  of  diene  conjugates (by  method  of spectrophotometry) and the Schiff’s bases (after D.I.Flether).

The influence of ozone on the above indices depended on the used concentration of ozone. The ozone concentration of 400 mcg/L induced a significant increase (by 2,6 times) in the antioxidative plasma activity. Meanwhile, the level of lipid peroxidation  molecular  products  had  a  tendency  to  decrease.  Higher  ozone concentrations (800 to 1200 mcg/L) led to reverse processes.

Introduction

For over 100 years ozone has been successfully used in various fields of medicine, but till 1993 in literature there were no references to ozone applications in the treatment of obstetric complications, that was why for safe clinical applications of medical ozone there was a need for experimental grounds of the present method of treatment. The aim of our investigation was to establish in vitro a range of ozone concentrations being safe for the homeostasis of patients with threatened abortion for the purpose of its further using in clinical conditions.

Materials and Methods

As a criterium in vitro for safety and efficiency of the given ozone concentrations it was considered intensification of the antioxidative defense system (AODS) and a decrease in the molecular products of lipid peroxidation (LP). The choice was based on the objectivity and informativity of the above indices as well as their importance for prognosis of obstetric pathologies including threatened abortion (2,3,5,6).

The Treatment of Endometritis in Combination with Ozone therapy

Abstract

The inflammatory diseases of female genital organs are numbered among the most frequent pathologies of fertile women. In order to increase the efficiency of  treatment and to avoid possible side effects we have used intrauterine irrigations with ozonated distilled water in case of  acute  endometritis. This method of treatment  has  been  provided  to 15  patients  with  postnatal  and  post-abortive endometritis. Based on the received results, we have drawn a conclusion that in the course of treatment all patients showed an improvement in general state, body temperature returned to normal, symptoms of intoxication decreased, sleep and appetite got better, pain syndrome disappeared faster, pathological discharges were eliminated. The results of laboratory investigations have demonstrated a credible decrease in ESR, normalization of leukocyte number, a decrease in circulating immune complexes.

Thus, for treatment of endometritis we may recommend the given method of ozone therapy to be used in combination with a basic anti-inflammatory therapy.

Introduction

Endometritis is being still numbered among the most frequent pathologies of fertile women. But  in  spite  of  considerable  success  achieved  in  the  treatment  of  inflammatory,  incl. gynecological diseases, the problem of efficient treatment of endometritis is not solved yet. It can be explained as follows:

  1. high frequency  of  complications  due  to  endometrial  manipulations (therapeutic abortion, diagnostic endometrectomy and so on) that mostly appear then in the form of acute endometritis;
  1. considerable risk that the infectious-inflammatory process localized in uterus will generalize;
  2. numerous side effects of medicines to be used within the traditional treatment of endometritis.

In practical gynecology along with a basic therapy (antibacterial, desensitization, vitamins, sedative remedies) antiseptic preparations have been widely used in the form of intrauterine applications. The aim of this manipulation is to improve the removal of pyo-necrotic masses as well as to produce a medicinal effect directly in the focus of inflammation that however
can cause a number of side effects.

Abstract

We observed 55 women which received adjuvant chemotherapy after non-radical surgery for ovary cancer, 35 of them underwent ozone therapy in the form of rectal insufflations of an ozone-oxygen mixture with assistance of the equipments and accessories of Medozons-Company according to a specially developed method.
The present investigation resulted in the following:
–     ozone therapy caused an improvement in patients’ state, in their sleep and appetite, a decrease in nausea and vomiting;
–     ozone therapy produced an immunomodulating effect, mainly on the humoral immunity;
–     ozone therapy improved the indices of lipid peroxidation without exogenous antioxidants.

Based on the above mentioned, we can draw a preliminary conclusion about the clinical efficiency and pathogenetic verification of ozone therapy in the correction of complications associated by chemotherapy of ovarian cancer.

Introduction

Systemic non-specific side effects of chemotherapeutics are frequently considered a limiting factor of further treatment and their efficient correction is the most important measure which is equal in its importance to determination of an adequate course of cytostatic treatment. In order to prevent chemotherapy complications it is necessary to carry out a complex of therapeutic measures of different action, in particular, metabolic treatment with medicines containing antidiabetic biguanides, hypolipidemics, deagregants, adaptogens, anticoagulants, bio protectants,  antioxidants (3).  This  method  has  some  disadvantages,  most  of  all – polypragmasy, which is doubly dangerous on the background of strong cytostatics used. (1).

In this regard, our attention should be drawn to non-medicinal methods of treatment which show high efficiency and cause minimum side effects. As the most promising we consider the use of numerous therapeutical, incl. immunomodulating properties of medical ozone (2,4,5) i.e. a strictly dosed gas mixture of purest ozone and purest medical oxygen.

The aim of an investigation is to study the efficiency of ozone therapy in the correction of complications associated by chemotherapy of ovarian cancer and to verify it pathogenetically.
